COVID 19 Safety Practices

The office is cleaned and sanitized regularly throughout the day and professionally cleaned at night.

We ask patients to reschedule appointments if they or family member have respiratory issues, fever, or returned from a high risk area in the past two weeks.

Please take your child’s temperature and any family member’s temperature prior your appointment. Please cancel your appointment if you are unwell.

Please minimize only essential people to attend the appointment.

All staff and providers take their temperatures daily.

If you prefer to pay over the phone prior to your appointment please call 480-659-5563.

It is not possible to conduct a reliable psychological evaluation for a child if they are wearing a mask. Individuals who are not being evaluated may wear a mask. The provider may wear protective face covering or a mask. This will be discussed with you prior to your appointment.

All reasonable attempts to limit physical contact with your child will be made.

Paperwork
In order to make an appointment you will to complete intake paperwork. You will be sent an email with your paperwork or it can be completed from the website. There is additional paperwork to be completed upon arrival to your appointment and we strive to make the most use of our time together.

Phone Calls
We receive a high volume of phone calls each day. Please leave a message and your call will be returned as soon as possible. We strive to return phone calls within 24 hours but typically return them in a few hours. Messages left on Fridays may not be returned until Monday.

Consent for Minors
Both biological parents will need to sign permission for the psychological assessment. Only one parent needs to attend the appointment but both will need to sign the permission form. If one parent has sole custody legal documentation will need to be provided prior to your appointment. If you have any questions about permission for minors please call 480-659-5563.

Cancellation/ Rescheduling
We strive to see individuals and their families as quickly as possible. Each appointment is a minimum of 2 hours for your individual assessment. If you no longer need your appointment please let us know as soon as possible so we may meet the needs of another individual. If you or your child is sick the day of your appointment please let us know and we will reschedule your appointment. No show appointments will not be rescheduled.

Assessment Services Only
No Counseling Services
We only provide psychological evaluations to help individuals and families receive accurate identification of psychological issues in order to develop appropriate and effective treatment plans. Many services that may be of assistance to individuals are not accessible without a psychological evaluation. We will put together recommendations and provide referrals based on the assessment results. We do not provide any counseling services.
